- NDIS participant number and plan information
- Health and disability-related information necessary to deliver your support
- Emergency contact information
- Financial information for billing and NDIS claims
How we use your information
We collect and use your personal information to:
- Deliver the NDIS support services you have engaged us for
- Communicate with you about your support plan and services
- Meet our obligations as a registered NDIS provider
- Comply with legal and regulatory requirements
We will never sell your personal information or use it for marketing purposes without your express consent.
Sharing your information
We only share your information where necessary: with the NDIS Quality and Safeguards Commission, with your nominated support coordinator, or with other service providers you have explicitly authorised. We do not disclose your information to overseas recipients.
